Chirurgická léčba vitiliga transplantací nekultivovaných autologních melanocytů - naše zkušenosti

Vitiligo patří mezi idiopatické poruchy pigmentace, kauzální léčba dosud není možná a konzervativní terapie je u řady pacientů neúčinná.
U pacientů s anamnézou neúspěšné konzervativní terapie je v řadě případů účinná terapie chirurgická. V posledních letech
se objevují různé modifikace chirurgické léčby od transplantace autologní kůže v plné tloušťce až po transplantaci samotných melanocytů
expandovaných kultivací. V článku je popsána chirurgická terapie vitiliga transplantací autologních nekultivovaných melanocytů
s použitím fibrinového lepidla jako buněčného nosiče. Popsaná metoda spočívá v odběru dermoepidermálního štěpu, jeho následném
enzymatickém rozvolnění na jednotlivé buňky účinkem trypsinu a aplikaci směsi buněk ve fibrinovém lepidle na depigmentovanou
kožní lézi připravenou k přijetí melanocytů ablací epidermis CO2 laserem. Touto metodou jsme ošetřili 13 pacientů se stabilním vitiligem,
k pigmentaci došlo u 5 pacientů.

Surgical treatment of vitiligo of non-cultivated autologous melanocytes transplantation - our experience
Vitiligo belongs to the idiopathic disorders of skin pigmentation. Causal treatment is impossible for now and conservative treatment
is ineffective with most patients. The surgical treatment can be effective with some patients with the history of ineffective conservative
treatment: Many modifications of surgical treatment of vitiligo have apperared during the past few years, from transplantation of
fullthickness skin grafts till transplantation of melanocytes expanded by cultivation. The surgical treatment of vitiligo by grafting of
autologous noncultured melanocytes in fibrin glue which is used as a cell carrier is described in this article. This method is based on
grafting of dermoepidermal graft and its incubation with trypsin. The skin cells suspension which is obtained after the trypsination
is then applied into a mixture with fibrin glue onto depigmented vitiliginous skin previously dermabraded by CO2 laser. We used this
method for treatment at 13 patients with stabile vitiligo, pigmentation was achieved at 5 patients.
