Acne and demodicosis are dermatoses with a similar clinical picture and a similar course. They even affect the same structures in the skin and, with a few exceptions, similar skin localities. Sometimes their symptoms may overlap in the same patient. However, the causative pathogen, the etiology of the disease, the main age group of the patients and basal effective treatment differ. Distinguishing between the two diseases in the routine outpatient setting may not be easy, especially in the examination of unremarkable to discrete lesions. This case report presents the case of a 39-year-old man who had long suffered from mild but recurrent facial papulopustular manifestations. Our task was to set the right diagnosis and the adequate treatment that would heal the torpid cutaneous symptoms. Taking into account the age of the patient, the nature of the primary and secondary efflorescences and, above all, the inefficacy of the applied treatment, a diagnosis of demodicosis was finally made.
