Chronická spontánní urtikárie léčená bilastinem

Chronická spontánní urtikárie (CSU) je charakterizována spontánně vznikajícími výsevy svědivých urtik a/nebo angioedému,
které se objevují v pravidelných intervalech a trvají déle než 6 týdnů. Průměrná doba trvání je 1–5 let, v těžších případech má
onemocnění tendenci přetrvávat i déle. Příčina zůstává ve většině případů neznámá. Infekce, zejména v oblasti horních cest
dýchacích a gastrointestinálního traktu, jsou v některých případech s CSU spojovány, ale poslední data ukazují, že tato asociace
je spíše koincidentální než kauzální. K aktivitě onemocnění může přispívat i intolerance pseudoalergenů v potravinách.
Základem léčby je u většiny pacientů symptomatická terapie nesedativními H1 antihistaminiky 2. generace (terapeuticky
prověřený cetirizin, levocetirizin, loratadin, desloratadin, bilastin, fexofenadin nebo rupatadin). Podle současných doporučení
je v refrakterních případech doporučováno zvýšení výše uvedených H1 antihistaminik 2. generace až na čtyřnásobek dávky.

Chronic spontaneous urticaria treated with bilastin
Chronic spontaneous urticaria is characterized by spontaneously arising itchy wheals and/or angioedema presents on regular
basis for longer than 6 weeks. The average duration of the disease is 1–5 years but is likely to be longer in more severe cases. In
the majority of cases, the cause remains unknown. Infections, especially those affecting the upper respiratory and gastrointestinal
tracts, have been associated with some cases of CSU, however, recent data indicate this association is more likely coincidental
than causal. The intolerance to pseudoallergens in foods may contribute to disease activity. Mainstay of treatment for the vast
majority of patients is symptomatic therapy with nonsedating H1-antihistamines (therapeuticly verified cetirizin, levocetirizin,
loratadin, desloratadin, bilastin, fexofenadin or rupatadin ). In refractery cases the current guidelines suggest an updosing of the
above mentioned nonsedating H1-antihistamine up to fourthfold.
