A growing body of evidence suggests that there are a variety of defects in the innate immune system that collectively affect the development and severity of atopic dermatitis. Genetically based impairment of the epidermal barrier, modifications of pattern recognition receptors and deficiency of antimicrobial peptides facilitate the entry of allergens and infectious microbes into the skin, where they encounter immunocompetent cells. The defects of innate immunity not only encourage the evolution of immediate type hypersensitivity to atopic allergens, but also explain susceptibility of atopic dermatitis patients to pathogens such as Staphylococcus...

Acne tetrada is a syndrom consisting of acne conglobata, hidradenitis suppurativa, perifolliculitis capitis abscedens et suffodiens and sinus pilonidalis. Perifolliculitis abscedens et suffodiens is a rare chronic dermatosis of unknown origin that affects hairy scalp and neck. It is a part of acne tetras and is present mostly in men over 20 years of age. Histopathology is necessary to asses the diagnosis. Peroral izotretinoin is the first choice of the treatment. Other therapeutic modalities include surgery, antibiotics, dapsone and biological therapy (anti- TNFα). A case of man treated succesfully with izotretinoin is described.

The increasing frequency of sensitization to gold especially in developed countries was noticed recently. The sources of sensitization are except for jewelry, dental materials and oral gold treatment also intracoronary Au stents and other implants. The papular dermatitis is the most common clinical picture especially on the face, eyelids, near the ears and on the hands. Typical lichenoid lesions on the oral mucous membranes or nonspecific stomatitis are frequently seen. Gold sodium thiosulfate 2% in petrolatum is recommended for patch tests with subsequent reading of skin reactions on day 2 and day 3 of exposure, with follow-up on day 7.

Skin metastases from gastric adenocarcinoma are rare, it has been reported in only 6% in males and 1% in females of all skin metastases (2). We report a case of 83 years old woman with relatively massive erruption of cutaneous metastasis from gastric adenocarcinoma, which was the first sign of this desease.
