Psoriasis of scalp, nails, palms and soles belong to group called difficult to treat psoriasis. Incidence of these types of psoriasis is not as frequent as typical plaque psoriasis, but it means a big psychosocial impact on the patient. For patient it presents limitation at work and daily routine. In dermatology in Czech Republic there are biologicals for treatment of psoriasis, chronic urticaria and melanoma available,and now also for treatment of atopic dermatitis- the first one is dupilumab. The introduction of biologicals into the treatment ofatopic dermatitis indicates the further release of new drug groups. The positive side effect is the revision of the existing drugs,especially ciclosporine A and methotrexate. This together could lead to treatment of more patiens with severe atopic dermatitisthan yet – the relative undertreament is a worldwide fact.

Psoriasis exhibits particularly high resistance to topical and sometimes even systemic therapy in some specific locations. Althoughthe affected area is not vast, there is often a dramatic decrease in the quality of life of patients.
